CSA RULES & REGULATIONS

WAIVER FORM POLICY

CSA requires EVERY player that participates in CSA leagues and events to submit a completed Individual Liability waiver form. This form covers any and all activities that the person participates in during the entire year.

A person needs to submit this form ONLY ONCE during the year, but without having a waiver on file, they will not be eligible to participate.

The line-up for every team for every game in every sport will be checked. If it is discovered that an ineligible player was used, that player is suspended from all CSA games for four (4) weeks.

The first time a team is found to have used an ineligible player, they will be warned (but the player(s) are still suspended). The second time a team is found to be in violation, that game will be forfeited. The third time, the team is ineligible to play the remainder of the season.

Additionally, any intentional violations of this policy will be grounds for immediate removal of a player and/or team from the league.

It is the responsibility of every captain to monitor their roster to make sure the players are eligible for CSA competition. The waiver form is available online and CSA will have blank forms at all fields.

The waiver form must be submitted PRIOR to the game, not afterward or the next day.

CSA has very liberal roster requirements, we allow teams to pick up players as needed, the only requirement we have is that they submit a COMPLETED liability waiver.

A complete form means just that. CSA requires an address and a contact method (either a phone number or active e-mail address). A form that does not have this information will not be accepted.


THESE RULES WILL BE STRICTLY ENFORCED AND ABSOLUTELY NO EXCEPTIONS WILL BE MADE TO THE ABOVE POLICIES.


CSA STAFF MEMBERS CAN NOT AUTHORIZE EXCEPTIONS.
